We will need the following information:

(If you want the shirt to be the colors in the picture please state that in the notes)
- Your Mascot
- Your Grandma name (Mimi, Nana, etc.)
- Name for sleeve (one sleeve only)
- Number (one number)
- Boy or Girl
- Team colors (Two colors only)

These shirts are unisex and they are not fitted.
They run a little big.
They are not pre-shrunk and will shrink a little after washing.
I am 5'4" 150lbs 36DD and I wear the medium.
